Description
PACLOVID is globally recognized as the latest and effective oral antiviral formulation against COVID-19. It is strongly recommended for patients with non-severe COVID-19 who are at highest risk of developing severe disease and hospitalization, such as unvaccinated, older, or immunosuppressed patients according to WHO data.
Action
Nirmatrelvir: blocks SARS-CoV-2 3CL protease, an enzyme that the coronavirus needs to replicate, and inhibits viral replication at a stage known as proteolysis, which occurs before viral RNA replication.
Ritonavir: Not active against SARS-CoV-2 3CL protease. It inhibits the CYP3A-mediated metabolism of Nirmatrelvir, thereby providing increased plasma concentrations of Nirmatrelvir.
Indication
Paclovid is indicated for the treatment of COVID-19 in adults who do not require supplemental oxygen and who are at increased risk of progression to severe COVID-19.
Dosage and Administration
The recommended dosage is 300mg Nirmatrelvir (two 150mg pink tablet) with 100mg Ritonavir (one 100mg white tablet) all taken together orally twice daily for 5 days. It should be given as soon as possible after positive results of direct SARS-CoV-2 viral testing and within 5 days of onset of symptoms. It can be taken with or without food. The tablets should be swallowed whole and not chewed, broken or crushed.

Precaution
Contraindicated in patients with hypersensitivity to the active substances or to any of the excipients (lactose).
Patients with severe hepatic impairment and patients with severe renal impairment.
Patients taking medicines which can inhibit or induce CYP3A enzyme, effecting plasma concentration of Nirmatrelvir.
As Nirmatrelvir is co-administered with Ritonavir, there may be a risk of HIV-1 developing resistance to HIV protease inhibitors in individuals with uncontrolled or undiagnosed HIV-1 infection.
Pregnancy and Breastfeeding
Pregnancy: No human data. Not recommended during pregnancy and women of childbearing potential not using effective contraception.
Breastfeeding: No human data. Breastfeeding should be discontinued during treatment and for 7 days after the last dose of Paclovid.
Fertility: No human data.
