Description
Tadalafil is a selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP) specific phoshphodiesterase type 5 (PDE5). When stimulation causes the local release of nitric oxide, inhibition of PDE5 by tadalafil produces increased levels of cGMP in the corpus cavernosum. This results in smooth muscle relaxation and inflow of blood into the penile tissues, thereby producing an erection. Tadalafil has no effect in the absence of sexual stimulation.

Treatment of erectile dysfunction
Dosage
The recommended dose is 10 mg taken prior to anticipated sexual activity and without regard to food. In those patients in whom tadalafil does not produce an adequate effect, 20 mg might be tried. It can be taken 30 minutes to 12 hours prior to sexual activity. The maximum recommended dosing frequency is once a day in most patients.

Contraindications
1. Hypersensitivity to Tadalafil
2. Administration of Tadalafil to patients who are using any form of organic nitrates is contraindicated since tadalafil may augment the hypotensive effects of nitrates.
3. Patients with myocardial infarction within last 90 days.
4. Patients with unstable angina or stable angina occurring during sexual intercourse.
5. Patients with New York Heart Association (NYHA) Class 2 or greater heart failure in the last 6 months.
6. Patients with uncontrolled arrhythmia, hypotension (<90/50 mm of Hg), or uncontrolled hypertension.
7. Patients with a stroke within last 6 months.
Precautions
Prior to initiating any treatment for erectile dysfunction, physicians should consider the cardiovascular status of their patients, since there is degree of cardiac risk associated with sexual activity. Tadalafil should be used with caution in patients who have condition that might predispose them to priapism (such as sickle cell anemia, multiple myeloma or leukemia) or in patients with anatomical deformation of the penis. Tadalafil should not be administered to patients with hereditary problems of galactose intolerence, the Lapp lactose deficiency or glucose-galactose malabsorption.
Pregnancy and Lactation
Tadalafil is not indicated for use by women. There are no studies of Tadalafil in pregnant women. There was no evidence of teratogenicity embryo toxicity or fetotoxicity in rats or mice, which received up to 1000 mg/kg/day.
Adverse Drug Reaction
The most commonly reported adverse reactions are headache, dyspepsia, dizziness, flushing, nasal congestion and back pain.
