Description

Carvedilol is a non-selective β-blocker with selective α1-adrenergic blocking activity. It reduces heart rate, blood pressure and cardiac contractility through non-selective β-blockade. It also enhances vasodilation via α1-adrenergic blockade, leading to reduced peripheral vascular resistance and an overall reduction in blood pressure.

Indications

Congestive heart failure

Left ventricular dysfunction following myocardial infarction

Hypertension

Dosage

Congestive Heart Failure: Initially, 3.125 mg BD for 2 weeks. Increase gradually to 6.25 mg BD at intervals of at least 2 weeks, followed by 12.5 mg BD, then 25 mg BD. Max: <85 kg: 25 mg BD. >85 kg: 50 mg BD, if condition is not severe.

Left Ventricular Dysfunction After Acute MI: Initially, 6.25 mg BD, increased after 3-10 days (based on tolerability) to 12.5 mg BD, and up to target dose of 25 mg BD. Lower initial dose of 3.125 mg BD may be used in some patients and rate of up-titration may be slowed.

Hypertension: Initially, 6.25 mg BD for 7 to 14 days. Increase to 12.5 mg BD if needed for 7 to 14 days, then to 25 mg BD if needed. Max: 50 mg daily.
